Register with the Surgery
To register with our Practice you must first live within our
practice area.
At Reception ask for ‘New Patient Registration Forms’ for however
many members of your family you wish to register.
All new patients are required to fill in a ‘New Patient
Questionnaire’ giving details of your medical history and lifestyle,
this enables us to book you an appointment before your medical
records arrive (which can take between 6 to 8 weeks), should you
need to be seen sooner.
